About the Role:
- We are hiring compassionate and articulate professionals to join our International Voice Process - Transitional Care Management (TCM) team.
- This role involves making outbound calls to recently discharged patients to ensure a smooth transition from hospital to home, provide post-discharge support, and help coordinate ongoing care.

Key Responsibilities:
- Make outbound calls to post-discharge patients to track recovery and offer guidance.
- Educate patients about medications, follow-up schedules, and home care instructions.
- Identify potential health concerns or red flags and escalate to clinical teams as needed.
- Maintain accurate patient records in CRM/EHR systems.
- Ensure adherence to HIPAA and healthcare data confidentiality standards.
- Coordinate with healthcare professionals for effective patient follow-up.
Requirements:
Experience: 1-2 years in International BPO / Healthcare / Voice Process (freshers with excellent communication skills can apply).
- Communication: Excellent spoken English with neutral or US accent.
- Strong empathy, listening, and patient handling skills.
- Proficient in computers, documentation, and CRM tools.
- Willingness to work in night shifts (US process).
- Immediate joiners preferred.
